To support the maintenance and health of older adults in the community, the Aging and Disability Resource Center (ADRC) puts on a Falls Prevention Program every year. This event is to help older adults become more aware of fall hazards in their home and as they travel outside of their home. Such things as hoarding, mixing drugs and alcohol, and pets are common fall hazards. These were discussed and showcased at the event. In helping with this program through my praticum, I provided best practice solutions to taking necessary steps in protecing themselves from falls in their home and outdoors. I provided them with several solutions to make changes to avoid unnecessary falls. I was also involved in setting up and working at the Falls Prevention program at the ADRC.

I learned to recognize several different economic, social and cultural backgrounds during this event, without prejudice or discrimination. This event was a cooperative effort of everyones work that was involved. It was done in a social environment.

Nutrition for Healthy Aging
Support the maintenance of health and wellness promotion for individuals and organizations
A nutritional evaluation was done on an older adult male. Body Mass Index was determined by height and weight, photos of food were taken and food labels to determine nutritional values. A plan with recommended calories and number of servings per food group needed were determined based on MyPlate recommendations.

While preparing for this photo essay, I demonstrated listening skills as I interviewed this cIient. I also used the APA formatting, as learned throughout my college experience to provide an evidence-based assessment on this client.

This photo essay proved to me that I had the skills to use technology in ways that I never had before. By adding photos to an essay, I learned of the tools and resources needed to produce desired results.
High blood pressure is a silent killer as there are no signs or sy​mptons. It is the leading cause of heart disease and stroke. As part of my praticum, I designed a presentation with goals of helping older adults learn the risks and causes of high blood pressure. This presentation was done at a local dining site for older adults. An explanation of the recommended blood pressure readings were discussed, along with how to read food labels and better understand sodium counts. The dietary approach to stop hypertension, or the DASH diet as recommended by MyPlate was explained along with foods to avoid to prevent high blood pressure.

This presentation taught me how to organize information in a way that everyone could understand. I took previously acquired knowledge and put it to use in a technological way that proved to be a new task for me.

Worksite Wellness Logic Model
Design, develop and implement health and wellness promotion activities and campaigns
This worksite wellness program was created with a classmate to get employees involved in eating "5 a-day" of vegetables or fruit. This wellness program provided them with motivation through meetings, incentives and education. All of this was done with evidence-based research throughout one semester of class.

In doing this project with a classmate, I gained responsibility for researching and taking action for my part in getting this project done in a timely manner. It took collaboration and communication skills, while learning and listening to each other's opinions and ideas. Prioritizing our meeting times to complete this project proved to be successful.
